Community Catalyst is committed to ensuring that Medicare beneficiaries receive stable, quality care they can count on. By promoting reform of the health delivery system for this vulnerable population, we can improve quality of care while lowering costs. Additionally, we believe it is important individuals and families be at the center of care planning and delivery and to coordinate care across medical and non-medical services—from acute to long-term settings.
