Roadmaps to Health Community Grants

A community’s health is shaped by more than just access to health care. Through the Roadmaps to Health Community Grants, Community Catalyst has partnered with the Robert Wood Johnson Foundation and the University of Wisconsin Population Health Institute (UWPHI) to support community based efforts to  break the cycle of poor health by improving education, financial opportunity, family stability and community safety.

Roadmaps to Health Community Grants supports two-year state and local efforts among policymakers, business, education, health care, public health and community organizations to address the social, economic and other factors that affect the health of people in their community. The program aims to create changes communities themselves have identified as ripe for action.

As the leader of the Community Grants program, Community Catalyst helps grantees achieve results in addressing the social and economic factors that affect the health of their communities. Grantees are assisted using a successful advocacy framework that includes coalition building and maintenanceengaging grassroots supportpolicy research and analysisdeveloping and implementing policy campaignsgenerating resourcesinfluencing decision-makerscommunications and media, and telling your story.

The Community Grants program is part of the County Health Rankings & Roadmaps Program, a groundbreaking collaboration of the Robert Wood Johnson Foundation and the University of Wisconsin Population Health Institute (UWPHI).

The County Health Rankings & Roadmaps program creates solutions that make it easier for people to be healthy in their own communities. Ranking the health of nearly every county in the nation, the County Health Rankings illustrate what we know when it comes to what's making people sick or healthy. The County Health Roadmaps show what we can do to create healthier places to live, learn, work and play. Having health insurance and quality health care are important to our health, but we need leadership and action beyond efforts to improve public health and health care.

Ultimately, a healthier America will be the result of people working together at every level—neighborhoods, municipalities, counties, states and the nation.
