Community Catalyst has released a new resource, Demographic Health Disparities and Health System Transformation: Drivers and Solutions, providing a background on the types of health and health care disparities currently affecting our country, detailing their drivers and summarizing some of the current and potential health system transformation options for addressing disparities.

Existing Quality Measures Not Necessarily Capturing the Benefits of Coordinated Care

A Center for Health Care Strategies brief discusses how existing quality measures in the dual eligible demonstrations and the Fully Integrated Dual Eligible Special Needs Plans (FIDE-SNPs) may not adequately pick up how these programs are improving people’s lives. Using individual success stories from Massachusetts and Minnesota, the brief demonstrates how current performance measurement tools may not capture the types of success that make better care possible. These conclusions have implications for quality measurement in all programs seeking to transform health care delivery and improve care coordination.
