Monday, Dec. 7 (1 - 2 p.m. Eastern) Webinar: Policy AGEnda: Partnering with and Supporting Advocates in Health Policy

The Hartford Change AGEnts will host a webinar that continues the conversation from a recent workshop at the Grantmakers in Aging conference. The workshop featured two national Change AGEnts organizations working to support policy leaders and advocates. This webinar will take a deeper dive into one of these organizations, Community Catalyst, and explore how advocates and geriatrics professionals are collaborating to amplify the voice of consumers in improving the health of low-income older adults. We’ll also take a closer look at how this type of collaboration in playing out in Rhode Island, where efforts are underway to better coordinate care for the state’s dually eligible beneficiaries.

Thursday, Dec. 10 (1 - 2 p.m. Eastern) Webinar: Building Population-Based Integrated Delivery Systems for Vulnerable Populations
