New Research Finds Well-Targeted Care Coordination Yields Return on Investment
A new report by Avelere for The SCAN Foundation makes the case for three best practice strategies on effective care management of high-risk Medicare beneficiaries:

- Improve data collection with a well-developed health risk assessment: health plans should invest in a comprehensive health risk assessment process which can help improve risk stratification and care coordination
- Implement targeted care coordination programs to drive outcomes and ROI: research revealed the potential for substantial cost savings when transitions are managed throughout the continuum of care
